Goddess Willow

One-to-one intake

This is the start of a conversation, not a sales form. A few questions about where your nervous system is right now, what pattern you want to work, and what kind of support you already have. There are no wrong answers. Some answers move us toward working together; some move us toward connecting you with someone in the Practitioner Referral Network who's a better fit. Both are good outcomes.

Nothing here is a diagnosis.

So Willow can write back. Stored only for the conversation that follows.

What would you like to be called?

Your pronouns.

Optional. They/them works as a default if you skip this.

What brings you to this work right now?

A few sentences in your own words. The pattern, the stuck place, the question. Whatever feels true.

How would you describe where your nervous system tends to live these days?

Best guess in plain language. Nobody scores this. It just helps Willow meet you where you are.

Are you in an active depressive episode right now, or working with a clinician for depression?

Shadow work pairs well with clinical care. It doesn't replace it.

Have you experienced a significant trauma in the last six months?

Acute event, loss, medical, relational, anything that still feels close.

Are you in active grief?

A death, a leaving, an ending. Grief wants ritual and time, not curiosity about hidden pleasure.

How often do you feel disconnected from your body, like watching yourself from outside or going numb?

Honest answer. This is not a test, and the answer changes what we offer first.

Some shadow material comes from things we had power over, and some comes from things done to us. Is the pattern you want to work with rooted in childhood abuse, assault, or systemic harm you had no power over?

We hold this question gently. There is no wrong answer. It changes whether we work together, or whether we connect you with someone trained in tending that kind of wound first.

For most of the last year, how often have you felt able to think, feel, and stay in your body at the same time?

A loose self-report on what trauma literature calls the window of tolerance.

Who is currently in your support system?

Therapist, partner, friends, family, group, spiritual community, nobody yet. Whatever is true.

You understand this form is the start of a conversation, not a guarantee of working together, and that Willow may refer you to a clinician in the Practitioner Referral Network if that fits better.
